Hazard Alert Reports

As part of the Injury and Illness Prevention Program, UCSC uses the Hazard Alert Report form to provide an anonymous method for employees to report safety hazards in the workplace, should they feel a need or desire to avoid speaking to their supervisor about the issue. Once these reports are filed, it can sometimes be difficult to find out what the final disposition was. Therefore, EH&S provides here the basic information from submittals which have been sent to our office, in hopes those who filed the forms will be able to see their concerns have been addressed.

June 23, 2006 McHenry Library public elevator When power goes off, elevator stops and doors will not open until fire department arrives. Occupants are stuck for up to an hour. The McHenry elevator functions correctly per its design, which acts to protect occupants from getting injured if they are attempting to exit when the power returns and motion may resume, and also to prevent forming a chimney in case of a fire.
September 22, 2004 Bridge on Heller Extension, south of North Remote parking lot Roadway is sunken below level of bridge surface concrete. Vehicles striking it may experience steering difficulties, and sidewalk presents trip hazard to pedestrians. Grounds repaired the change in elevation by adding asphalt to the roadway and sidewalks to make a smooth gradient over the transition areas.
September 21, 2004 New Engineering building and lecture hall next to JBEB Clear glass panels next to glass doorways are misleading and numerous people are walking into the solid surfaces thinking they represent an exit point. PBSci Facilities has placed blue warning tape across the open window spaces and is working at planning a permanent method of providing a safety warning which will not unduly disfigure the buildings.
March 17, 2004 Merrill College A and B dorms Walkway carpets on roof deteriorated and shifting, causing trip hazard with significant fall potential Work order was put in for carpentry shop to remove old carpets from roof.
March 17, 2004 Earth and Marine Sciences building freight elevator Doors close too quickly and do not stop when person or object is in the way EH&S and elevator maintenance supervisor inspected all elevators in EMS and were unable to duplicate or observe the reported problem. All elevators were functioning per normal operating specifications.
March 11, 2004 Porter College Hungry Slug café Grease dripping from grill ventilation duct on sidewalk on south side of building is causing slip hazard Notation regarding duct and sidewalk cleaning being required was included in quarterly inspection of cafe facility. College maintenance steam cleaned sidewalk and duct.
March 4, 2004 Porter College room 250 Electrical shock hazard from the track light on top of the Media Services cabinet Media Services removed the light.
March 1, 2004 Cowell College Turner Hall dorm mechanical room Water heater has been installed too close to electrical supply panels EH&S concurred with assessment of hazard and forwarded request to move either the water heater or the electrical panel. CUHS initiated project to move water heater which has been completed.
January 20, 2004 Jack Baskin Engineering Building loading dock No ramp available for moving materials from parking lot up to dock level, possible lifting injury hazard Recommendation forwarded to building owner to repair broken concrete step by side of loading dock. Several alternative routes for moving materials to dock level proposed in letter to Physical Plant director for consideration.
July 7, 2003 Social Sciences 2, room 181 90 degree-plus heat due to lack of ventilation and large number of computers/servers No OSHA standards for indoor temperatures, campus goal is meeting ASHRAE guidelines of 68-78. Suggest working with HVAC department to improve overall ventilation and exhaust to remove heat generated by equipment. On hot days, employees take measures such as relaxed dress code (shorts, sandals), use fans for air movement, take frequent breaks and drink cool water, and relocate to other areas or spaces to perform work.
January 22, 2003 Raised pink walkway to staff door, lower west entrance to Science Library Puddles collect on plywood walkway from rain and can freeze into sheets of ice, causing a slipping hazard Grounds department agreed to drill holes in plywood flooring of walkway to drain it and prevent accumulation of standing water
October 8, 2002 Dolphin pool sump pump area at Long Marine Lab Pump putting water onto steps and pathway, allowing build up of algae causing a very slippery surface Paint shop determined area too wet for non-skid coating, work order forwarded to carpenters to install cleats on stair treads
September 13, 2002 East Fieldhouse boiler room off pool deck Storage of chairs and miscellaneous equipment making access to electrical panel impossible Items were removed from area near breaker panels
July 25, 2002 Thimann main mechanical room, chemical scrubber area Scrubber equipment not being maintained, guage failed and leaking at top of unit, pump leaking, empty chemical drums and trash piled around on floor Reported to owner of scrubber, EH&S to follow up
April 9, 2002 Intersection of Heller Rd. and the entrance to the west perimeter parking lot Entrance is too narrow, vehicles making turn from Heller to West Remote parking lot are in danger of hitting vehicles extended into intersection to make left turn from parking lot access road on to Heller. Also, the roadway edge is broken so that drivers cross into the other lane to avoid tire damage, causing more close encounters. Facilities Project Request was filed to repair and resurface the road to address width of access through the area. A housing addition planned nearby will realign the road and planners have said the design of the intersection as a whole will be addressed at that time.
