A Researchers Guide to the Controlled Substance Program The University of California, Santa Cruz (UCSC) Controlled Substance Program describes the special requirements and procedures applicable to the procurement, storage, use, transfer, disposal, and inspection of Federal Drug Enforcement Agency (DEA) Controlled Substances used for research and teaching activities at all UCSC locations. Federal and state law regulates the manufacture,
distribution, use, storage and disposition of controlled substances. Controlled
substances generally include narcotics, stimulants, depressants, hallucinogens,
anabolic steroids, and chemicals used in the illicit production of controlled
substances. Procurement Process Controlled Substance Procurement Flow Chart Forms Controlled Substance Authorization - To be completed by each person with access to or contact with controlled substances. Controlled Substance Use Authorization - To be completed for each project/protocol using controlled substances. Includes information on the controlled substance(s) used, the bona fide justification for the project, the storage location, and authorized personnel. Controlled Substance Use Log - To track the use of controlled substances. Each container of controlled substance received should have an associated use log. The log should be kept in the secure storage location with the controlled substance. Transfer Log - To track the transfer of controlled substances from one authorized person or location to another.
