Laboratory Hazard Assessment

Assessment, a UC-wide online tool supporting laboratory safety, is available for Principle Investigators, facility managers, and lab workers. Assessment was developed as part of the Personal Protective Equipment policy signed by outgoing President Yudof (effective March 31, 2014). Assessment provides a platform for Principal Investigators (PI) to identify laboratory hazards and specify required personal protective equipment (PPE).

Assessment facilitates PPE specific training and distribution. The tool also provides a convenient way for labs to track their current members, including records of their training.

Please use Assessment to identify hazards and determine what PPE should be used to safely conduct work.

Assessments are specific to each lab and organized under several general categories (chemical, biological, physical, ...).

Additional PPE oriented training covers equipment currently in use at UCSC.               Lab coat(s) and safety eyewear can be obtained after the hazard assessment has been reviewed and the training completed.

Process

New Users

1. Log into Assessment (https://ehs.ucop.edu/)

2. Update user info - Select you Title (undergraduate, staff, PI, ...) and Primary Appointment (funding Division).

3. Have your PI or delegate add you to the lab group.

Principle Investigator

1. Create a lab group - Give group a name, identify group members, and select the locations covered.

2. Assign a delegate(s) -  Delegates can be designated by editing lab member permissions in Manage Lab.

3. Manage roster - Add and remove people from your lab. Room allocations can also be modified. 

4. Hazard assessment - Create, or modify, a custom hazard assessment for this group.

5. Finalize hazard assessment - the assessment must be reviewed and certified by the PI before group members can review.

Delegates

1. Manage roster - Add and remove people from your lab. Room allocations can also be modified. 

2. Hazard assessment - Create, or modify, a custom hazard assessment for this group.

All Users

1. Review the hazard assessment - Anyone working in the lab must confirm that you have read and understand the lab hazards.

2. Training - Each lab group member, including the PI/supervisor, will need to complete a brief training and quiz on the use of PPE.

3. Obtain PPE - Group members and PIs will need to visit the PPE Office to obtain fitted PPE. Researchers are encouraged to select an appointment time on the PPE calendar; PPE office information can be found here.
